It is the knowledge you need to operate your processes and achieve the intended results of your QMS. Clause 7.1.6 says it is to be retained, applied and shared to the extent necessary, and to consider how to acquire or access additional knowledge when needs and trends change.

Is a knowledge register mandatory?

No. The standard does not mandate a specific document. A register is a practical way to keep a record of what you determined and how it is retained and shared.

How is this different from a training matrix?

A training matrix tracks the competence of people. A knowledge register tracks the knowledge the organization would lose if those people left, and how you are protecting it.
