ISO 13485 Certification Cost for a Small Company

By Ishan Maharaj, Founder and Principal Consultant. Last reviewed 26 October 2026.

Short answer: the cost of ISO 13485 certification has four parts: certification body audit fees, preparing the quality system (templates, consultant or both), your own staff time, and the product evidence the standard expects, such as design files and process validation. The audit-day starting point is published: three days for a company with one to five people, and four days for six to ten. Each certification body multiplies those days by its own rate, so request written quotes. Our consulting rates are published below.

How many audit days does ISO 13485 need?

IAF Mandatory Document MD 9 (Issue 4, Version 2, 2023) sets initial audit time for medical device QMS audits. The table gives Stage 1 plus Stage 2 days by effective number of personnel.

Effective number of personnel Initial audit (days)
1 to 5 3
6 to 10 4
11 to 15 4.5
16 to 25 5
26 to 45 6
46 to 65 7
66 to 85 8
86 to 125 10

These are starting values. MD 9 allows more time for factors such as device complexity, reliance on critical suppliers, multiple shifts or production lines, or poor regulatory compliance. It allows reductions, capped at 20% combined, where the scope excludes manufacturing or the product range or design and production processes have reduced. If your certification scope covers only distribution or transportation, time may be reduced by up to 50% in total. If you certify ISO 9001 and ISO 13485 together, at least 25% is added to the minimum days.

What does the consultant part cost?

Published remote consulting prices (USD, excluding taxes) from our consulting page:

Service Price
Advisory hours (two-hour minimum) $125 per hour
Remote internal audit, single process from $1,250
Design controls and DHF support from $4,500
Audit readiness sprint from $4,500
ISO 13485 framework development from $6,500
ISO 13485 framework and implementation from $12,500
ISO 13485 full build and audit readiness from $22,000

What do templates cost compared with a consultant?

Templates are one-off purchases. The ISO 13485 Starter Kit ($249) holds the six core kits: gap assessment, quality manual, document control, CAPA, internal audit and training. The Complete QMS Kit ($449) holds every ISO 13485 kit we make, including design controls, risk management, supplier control, process validation and complaint handling. Templates still need an owner to adapt them to how you work, which is internal time.

How long does it take?

Greenlight Guru, an eQMS vendor, reports that most medical device companies become certified in six to 18 months, depending on the state of the quality system and certification body availability. Treat it as an industry estimate rather than a promise. The main variables are how much of the system exists, whether product evidence (design files, validations) is complete, and when your certification body can schedule audits.

Worked example: a 10-person manufacturer

  1. Audit days: MD 9 gives 4 days for the initial audit. Ask two accredited bodies to quote against 4 days, stating your scope and device risk class.
  2. Gap check: run the Gap Assessment Workbook ($49) across 71 requirement areas, or buy a remote internal audit from $1,250.
  3. Build: adapt the Starter Kit or Complete Kit. Allocate named owners for design controls, CAPA and document control.
  4. Review: book advisory hours at $125 per hour or a fixed-scope document review before Stage 1.
  5. Add your staff hours and compare with a consultant-led build.

The scenario is illustrative; costs differ by scope.

Which route suits a small company?

  • Templates plus targeted help: you have a quality or regulatory person with time. Lowest cash cost.
  • Consultant-led framework: nobody owns quality, or a customer or tender deadline is fixed. See ISO 13485 consulting.
  • Distributor-only scope: MD 9 allows reduced audit time for distribution or transportation-only scopes, so check how your scope is written.

Frequently asked questions

Does the certificate cost include FDA or EU MDR fees?

No. ISO 13485 certification is issued by a certification body. Regulatory submissions and notified body fees are separate and not covered on this page.

Do I need ISO 13485 if I only distribute devices?

It depends on your customers and your market. Under EU MDR, distributors must meet Article 14 duties, which do not themselves require a quality management system, but many customers and tenders ask for ISO 13485. See ISO 13485 for distributors.

Is the certification body's fee fixed by the standard?

No. The audit days are guided by IAF MD 9; the price per day is set by each body.

Can a consultant guarantee we pass?

No. The audit decision belongs to the certification body.
